1. Client Information

  • Company Name: Fill in the name of the company or entity submitting the samples.
  • Contact Name: Enter the name of the primary contact person responsible for the submission.
  • Email Address: Write a valid email address for communication.
  • Phone Number: Include a phone number for follow-ups or clarifications.

2. Sample Submission Details

  • Lab ID: Verify that the lab IDs match the intended sample.
  • Sample Source: Indicate the origin of each sample (e.g., "Sink", "Water Fountain").
  • Batch ID: Assign a batch ID corresponding to your internal tracking.
  • Time Collected: Record the exact time when the sample was collected.
  • Requested Analysis: Specify the type of analysis requested (e.g., "THC Content").

3. Editable Fields for Additional Data

Complete any blank fields such as "Collection Site" or "Collection Date" before submission. Ensure accuracy when filling out.

4. Notes Section

Add any relevant notes about the samples:

  • Storage conditions: e.g., "Kept at 4°C."
  • Special handling instructions: e.g., "Handle with gloves only."

5. Signature Section

  • Receiver's Signature: The individual receiving the samples should sign to confirm receipt.
  • Date: Enter the date when the samples were transferred.
  • Temperature: Record the sample temperature at the time of transfer.
  • Received By: Include the name of the person who received the samples.

6. For Lab Use Only

Leave this section blank for the laboratory to complete upon receiving the samples.

7. Review Before Submission

  • Double-check all fields for accuracy and completeness.
  • Verify that signatures are included where required.
  • Confirm that the COC form matches the samples being submitted.

8. Submit the Document
